Do you need to undertake a practicum at this stage?<br>

Have you previously completed Introduction to Libraries (now defunct unit) and not completed a first year practicum? YES/NO<br>

Are you currently enrolled in LIS330/INFM130 Information Theory & Research in SP3, 2014 or Information Theory & Research 302/502 in semester 2, 2014 and not yet completed a practicum? YES/NO<br>
<br>
Are you enrolled (or intending on enrolling) in LIS330/INFM130 Information Theory & Research for SP1, 2015 or Information Theory & Research 302/502 for semester 1, 2015? YES/NO<br>
<br>
Are you enrolled in the Masters in Information Management (MIM), undertaking the dual qualification in librarianship AND records management/archives, AND required to undertake your second practicum before SP1 or semester 1, 2015? YES/NO<br>

If none of the above, are you intending to complete your course this year AND have yet to complete your practicum requirements? YES/NO<br>

If you answered YES to the first question you will need to complete a two week full-time placement. Please submit your paperwork.<br>
<br>
*Please note that the bulk of first year placements for both OUA </span><span style="font-family:"Calibri","sans-serif"">students and Curtin students will be organised for the OUA SP2, in particular July and for the Curtin mid-year break which is July, after
students have completed the unit Introduction to Librarianship. A call for prac applications for undergraduate and post-graduate mid-year placements will be made in March 2015.<span style="color:black"><br>
<br>
If you answered YES to any of the subsequent questions then you are required to undertake a three-week full-time practicum (third year, grad dip and MIM students). Students need to make arrangements for leave from work and/or childcare in order to undertake
the practicum. However, there may be some flexibility for exceptional cases where it is not possible to undertake the practicum full time or during the time periods outlined in the Practicum Manual. Please contact the Practicum Coordinator as soon as possible
should you have any queries or concerns regarding these requirements.<br>

3. Curtin Insurance Information Sheet (please read this to ensure you are familiar with the coverage)<br>

Please read the Practicum Manual very carefully, particularly the section on 'Procedures for Organising Placements' as it provides detailed instructions on the application process. You must complete and return the Practicum Application Form, a CV and the Contact
Details Spreadsheet. It is important that you read all instructions carefully and complete and return all forms correctly so as not to delay the organisation of your placement. Incomplete or incorrect forms will be returned to you for correction, which may
mean missing out on your preferred placement venue. ALL completed documents MUST be emailed as attachments to
<a href="mailto:prac.infostud@curtin.edu.au" target="_blank">prac.infostud@curtin.edu.au</a> from your Curtin email address. The Application Form should be completed electronically i.e. complete the details within the attached Word document. Please note you
must follow the correct procedure for subject lines of emails and file names of all attachments; this is outlined in the Manual.<br>

If you wish to apply for an exemption from, or variation to, the practicum placement based on your work experience, you must submit an application by Friday 26 September 2014. Please refer to the 'Exemptions and Variations' section of the Practicum Manual for
the criteria and guidelines for applications. If applying for an exemption, you must enrol in the relevant unit anyway as there are other coursework requirements to complete.<br>
